Insurance & Self-Pay Options
We are an out-of-network provider currently accepting insurance.
Your insurance policy may or may not include acupuncture coverage, and we can help you find out if your plan covers our services. Complete the INSURANCE VERIFICATION FORM and we can begin the process of verifying your insurance benefits. Some of the answers that we will obtain for you are:

※ Do you have in or out-of-network coverage?
※ Do you have an annual deductible?
※ Do you have a co-payment or co-insurance?
※ Do you have an annual visit limit?
Please note verification of insurance does not guarantee coverage of acupuncture services. To best serve you, we invite you to verify your insurance coverage HERE or by contacting us before your first scheduled session. The time-of-service cash rate will be charged if your insurance benefits are not verified before hand.
Cancellation and Payment Policy
Payments: Payment is due at time of service | Payment Type: Cash, MasterCard, Visa, Flexible Spending Accounts (FSA) , Health Spending Account (HSA)
Non-Refundable Payment Policy: All services are non-refundable. No refunds will be provided for the full or partial price for any used products or services.
Refund Policy On Pre-Paid Packages: Refunds are acceptable for “discount prepayment packages”. If you do not complete the “course of treatment” (example; 6 sessions) the discount no longer applies. When refunded you will be charged at full rate for each treatment you have used and be refunded the remainder.
Cancellation/Late Policy: Kindly, provide at least 24 hours notice prior to cancelling your appointment to avoid being charged the full session price. If you are more than 15 minutes late to an appointment, your session time will be shortened or you may be invited to reschedule your appointment.
